To ensure your child is eligible to participate in CHS athletics, please complete the following forms and return to Coach T (tristan.winkelman@chsaustin.org) as PDFs
1) TMS App Completion: see video below
2) TAPPS Physical: A current physical examination dated within the past year
3) Medical History Form
4) Signed CHS Athletic Handbook
5) Team Registration: Complete the Google Form for any sport your athlete plans to participate in.

Please provide all information required on the sports sign-up, read and agree to the enrollment acknowledgement, and submit to officially register your son or daughter.
